The Assisted Living Waiver Program (ALWP) is a state initiative developed by the Department of Health Care Services (DHCS) aimed at supporting individuals in receiving the same level of care as in nursing facilities, but in an assisted living environment. This program facilitates the transition from nursing facilities to a more community-oriented setting, promoting greater independence and quality of life.
To qualify:
A state-approved Care Coordination Agency (CCA) in your county will conduct a thorough assessment for individuals seeking services. This includes interviews and assistance with the application process and necessary paperwork.
Once the assessment is complete, individuals must find and relocate to an ALW-approved facility (RCFE) for the program to be activated.
